The HealthSource of Ohio Mobile Dental Team will travel to Bethel-Tate Schools soon to provide dental services to students enrolled in the mobile dental plan. To enroll, please complete the permission packet that was recently sent home with your child. You can also access the permission packet by scanning the QR code pictured in the flyer or by visiting https://www.healthsourceofohio.org/services/mobile-dental/. Packets must be completed and returned by February 17th.

An exam by a dentist
X-rays
Cleanings
Sealants, if needed
Fluoride, if needed
Summary of the visit to take home
Goodie bag, including toothbrush, toothpaste, and dental floss
